WHAT IS PREMIERE CARE?
EnVogue Med & Wellness is introducing "Premier Care" to the Chicagoland area. This new brand of patient membership ensures access to an experienced primary care and weight management provider whose panel is much smaller than traditional practices. This translates into easily attainable, unrushed appointments when you actually need them. Also, many of your medical needs can be efficiently addressed utilizing secure multi-channel platforms eliminating the need to come in for a visit. Imagine being able to text your doctor for a refill!

Also, our patients who see us for both primary care and weight management pay only ONE membership rate!

WHAT IS ENVOGUE VALUE SERVICE?
With health care costs out of control, EnVogue Med & Wellness seeks to continually provide personalized high-quality services at reasonable, transparent prices for self-pay patients compared to other large health systems or big-box pharmacy chains. In addition, if your physician recommends labs, tests, or imaging we can assist our patients, upon request, in finding the best price without sacrificing quality.

DO YOU ACCEPT MY INSURANCE?
Yes, we now accept Medicare Part B, BCBS and Anthem PPO/POS, BCBS PPO/POS, Aetna PPO/POS, UnitedHealthcare PPO/POS, and Cigna PPO/POS. At this time, we do NOT accept Medicare Advantage, any HMO, or Medicaid. For self pay members, you can choose between DPC or premiere care.

CAN I USE FSA/HSA FUNDS FUNDS FOR YOUR SERVICES
We recommend you check the latest IRS and state rules to officially answer this question for the current and upcoming year. Historically, membership fees have been considered retainer fees, therefore, not eligible as an FSA/HSA expense. However, medical services (i.e. office/telemedicine visits), labs, tests, medications, and imaging have been eligible FSA/HSA expenses. Also,
Health Insurance Cover Membership Fee does not cover Medicare.​
